Immunization Form

Since our school is headquartered in Georgia, per the Georgia Department of Public Health, Georgia law requires children attending a school be protected from certain vaccine-preventable diseases. The immunization requirements for school attendance are outlined in the Georgia Immunization Certificate (Form 3231) and the Policy Guides 3231INS and 3231REQ.

If you have questions, please talk with your healthcare provider.

Your healthcare provider will have their own certificate of immunization. Before the first day of school, please have your healthcare provider fill out an updated certificate of immunization and upload a photocopy/scan of that certificate below.
