Thank you for taking the time to fill out the Media Permission, and Technology Agreement for the 2021-2022 school year.

If you have a child/children in the Elementary Studio, please have them review and E-sign this Elementary Technology Agreement prior to completing the form below.

If you have a child/children in the Middle or High school Studio, please have them review and E-sign this Middle and High School Technology Agreement prior to completing the form below.

We would like to tell the community about the many positive things taking place at The Forest School. We respect the wishes of parents/guardians who do not want their children photographed or videotaped, or their child's name, work, or photo displayed on social media or our school website. Photos, videos, interviews, or student work and names will only be displayed by Guides and Co-founders and will not be used for commercial gain. Please check all that apply.

I consent to my child(ren) being photographed, videotaped, or interviewed and their name and image used in school publications and media coverage of school related events.

I consent to my child’s/children's school work (text, videotape, audio, art etc), name, photo/image/video image being used on the school website and social media sites.

Appropriate use of technology agreement

Please review the Elementary Technology Agreement (https://www.theforest.school/elementary-technology-agreement) and/or Middle School Technology Agreement (https://www.theforest.school/middle-school-technology-agreement) with your child, discuss it, have your child/children sign it, and then check the box below.

Electronic Signature Agreement. By entering your name and selecting the relevant Media and Technology buttons, you are signing this Agreement electronically. You agree your electronic signature is the legal equivalent of your manual signature on this Agreement. You consent to be legally bound by this Agreement's terms and conditions. You further agree that your use of a key pad, mouse, or other device to select an item, button, icon or similar act/action, or in accessing or making any transaction regarding any agreement, acknowledgement, consent terms, disclosures or conditions constitutes your signature (hereafter referred to as "E-Signature"), acceptance and agreement as if actually signed by you in writing. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature or any resulting contract between you and The Forest School. You also represent that you are authorized to enter into this Agreement for all persons who own or are authorized to access any of your accounts and that such persons will be bound by the terms of this Agreement. You further agree that each use of your E-Signature constitutes your agreement to be bound by the terms and conditions of The Forest School Agreements as they exist on the date of your E-Signature.
